Heralding from a realm of madness and nightmare, these were created from the first mortals to enter that realm, who were absorbed by a nameless entity and transformed into the first kaortis. They traveled back through the portal and destroyed it, stranding themselves on the Material Plane. They are humanoid in shape but are nonhuman in appearance.
