These occur on occasion when expectant mothers are transformed by ghouls, lacedons, and ghasts into undead. They tend to be spiteful and self-loathing, having been raised among ghouls and trapped by their desires for living flesh. This acquired template can be applied to any giant, humanoid, or monstrous humanoid creature.
